Asthma-Friendly Schools Initiative Champion Awards Application Checklist

Recommended Application Preparation

  • Learn more about the Asthma-Friendly Schools Initiative (AFSI) and how it supports a healthy school environment for students, faculty, and staff.
  • Familiarize your Asthma-Friendly Schools Initiative coalition with the strategies and recommended components listed in the Asthma-Friendly Schools Initiative Toolkit and compare them with the activities conducted in your school. The strategies outlined in the Asthma-Friendly Schools Initiative Toolkit include master planning, maximizing health services, providing asthma education, creating an asthma-friendly school environment, and encouraging physical activity when students with asthma are able.
  • Participate in the Asthma-Friendly Schools Initiative Champion Awards webinar to learn more about the awards program criteria as well as examples of schools implementing a comprehensive approach to asthma management.
  • Contact your local Lung Association office with your intention to apply. They may be able to provide additional resources and support as you complete the application process. Find your local Lung Association at www.Lung.org or call 1-800-LUNGUSA.
  • Request that colleagues and partner organizations review the application for completeness and the general cohesiveness of narrative portions.

Instructions to Complete Application Process

  • Applications must be completed by a representative of a primary or secondary public, private, or charter school.
  • Obtain a letter of commitment from the school principal to ensure their support in healthy, asthma-friendly learning environments.
  • Complete and review all narrative portions of the application.
  • Compile all required components of the application, including the application form, supporting materials, and the letter of commitment.
