Responsibilities:

The Program Coordinator works under the guidance of the Director of the Chronic Respiratory Disease Section of the Indiana State Department of Health.  This position assists with grant development, program planning, grant reporting, and sub-grantee monitoring; works closely with all program staff and partners to ensure grant related activities are completed in accordance with program work plan; interacts with community, professional organizations; and Indiana Joint Asthma Coalition (InJAC) member organizations to develop partnerships and assist with systems, environmental, and policy change focused on reducing the burden of asthma in Indiana.

Responsibilities include but are not limited to:

  • Assists the Program Director in developing and evaluating the asthma health communication plan and responsible for implementation of the plan.
  • Assists the Program Director in preparing and submitting plans and reports to CDC and other funders accurately and on time.
  • Collaborates with internal and external agencies/organizations to provide technical and other assistance with systems, environmental, and policy change.
  • Develops, evaluates, and seeks new chronic respiratory disease education materials and program information, and maintains and updates diabetes resources as needed.
  • Assists with work plan development and grant writing.
  • Disseminates chronic respiratory disease related information to partners, other professionals, and the general public as needed.
  • Serves on internal and external committees and task forces.
  • Submits reports to chronic respiratory disease section director on a timely basis
  • Performs other duties as required.

Qualifications:

  • Bachelor’s Degree in Public Health, Community Health, Health Education, Project Management  or related field preferred. Master’s Degreee preferred
  • Knowledge of population-based activities, from planning through evaluation, to ensure successful project outcomes
  • Knowledge of health communication strategies and plan development
  • Knowledge of asthma and other chronic respiratory diseases
  • Knowledge of evidence based best practices for reducing the burden of asthma in Indiana
  • Highly organized and able to manage multiple projects at a time
  • Experience with training and public speaking
  • High skill level with all Microsoft Office software products
  • Excellent written and verbal communication skills
  • Ability to work cooperatively with national, state, and local stakeholders
  • Ability to work both independently and as an integral member of a team

Other Qualifications:

I. Difficulty of work

  • Incumbent utilizes planning, implementation, and evaluation to assist with compliance of the federal asthma program cooperative agreement.  Incumbent performs duties within the policies of the ALAUM, ISDH and requirements established by federal and state agencies, including those mandated by the Indiana General Assembly.
  • Work is broad in scope with many variables that must be interpreted.
